Upon receipt of the membership fee (Activation Date): Your membership begins and will automatically renew every 12 consecutive months. If payment is not renewed on the 1-year anniversary of the activation date, the plan will automatically terminate.
Appointment Cancellation Policy: Patients who do not provide at least 48 hours' notice to cancel a scheduled appointment will be charged a $50 cancellation fee**.
**Membership Guidelines: This plan cannot be used in conjunction with dental insurance or third-party financing.
Automatic Renewal: The annual membership fee renews automatically until canceled by the patient. No refunds of membership fees will be issued if the participant chooses not to utilize the plan or if benefits remain unused.
